Unfortunately that wasn't it. I'm on Windows for this sandbox env, am the admin of the machine, run the Apache service, and have full control over the Mantis directory.
For now I blew away my Mantis install/database and reinstalled everything. If I wasn't in a position to have done this clean install I would pursue this further with you.

I'm not sure what you mean by "before changing the PHP file". In my case the steps I've followed for upgrading Mantis are:
1- click Plugin Manager's "Upgrade" link in existing 1.1.4 installation
2- download 1.1.5 and unzip in a folder beside my current 1.1.4 installation
3- run ~/mantis/admin/install.php in 1.1.5 installation
4- unpack PluginManager on top

Do you have notes anywhere that document best practices for managing your plugins when upgrading Mantis?
In a sandbox I've upgraded Mantis from 1.1.4 to 1.1.5 following their notes (basically just unpack new Mantis beside old Mantis and copy over config_inc.php). Should I be able to install the plugin-manager and the plugins in this new 1.1.5 version just as I did with the 1.1.4 version

i feel like i've been nothing but trouble as a new user i hope at the least i'm helping identify real problems.
i get the following errors trying to install the following plugins. no other information shows up in the install report.
MyView 0.0.4
Install the plugin Step 2 / 2: Create table FAIL
Restore old files FAIL unable to restore
AutoFill 0.0.4

that fixed the problem. i successfully installed the updated plugins, was able to configure them, and use them as expected.
thanks so much for the quick response and fixes.
the community of mantis, especially you and and your quick responses, are likely what will tip us to go with Mantis instead of the other bug-tracking systems we're evaluating.

i've been investigating more of your wonderful plugins recently, but have run into an issue where two of them failed during installation, but are listed in the Mantis app, and are not uninstallable.
specifically, CFNotes and LongCF failed, and here is what i see in the plugin list:

yes - it is resolved.
the plugin manager is very nice btw. i like the convenience of being able to link straight to the plugin list and just click a link to install another one.

It looks like this has been mentioned several times in earlier posts, but they were able to resolve their issues. I have not been so lucky. I am running Mantis 1.1.4 on Windows XP. I have tried to install the PluginManager versions 0.4.0 and then 0.3.0 both without success.
With .0.3.0 I get similar errors to earlier posts.
With 0.4.0 I get 2 red error messages (Install the plugin Step 2 /
